PRP Week 18 - Clinical (Ward 8D)


Hurrayyyy~ Finally I entered into clinical department. The toughest and the most exhausting dept ever.

Since this week was started with CNY breaks, thus I only attached to Ward 8D for 3 days only. Ward 8D is a paediatric ward which means the patients are mostly children. However, there is a case of patient age 15 years old who was admitted here because other ward didn't want to admit him.

During my attachment to ward, there are some routines had to be done throughout the day. These are what most of the ward pharmacists will do:

1. Clerk case in the morning to see whether there are new admissions into the ward.

2. Write down all the medications for each patients and their significant lab findings to see whether there is a need to make interventions.

3. Follow specialist ward round, usually started at 8.30 am or 9 pm depending on the specialist. Most of the time, house officer (HO) and medical officer (MO) will follow the round and they will present the case to the specialist. The specialist then will do some examination on the patient such as blood pressure measurement, pulse rate, saturation and others. Usually my job is to listen to the discussion and then write down if there's any change, addition or removal of medications. Also need to write down which bed will be discharged today. Any inquiries regarding medications will usually be asked to the FRP since they have more experiences.  

4. After round, I will go to Farmasi Satelit to check the medication trolley for the ward. If there's a change, I will checked the trolley, removing or adding the respective medications. However, there's no trolley for ward 8D, so I'm kinda free after ward round.

5. Since some paeds wards do not have medication trolley, I have to write down a list of patients who have CIVAS and then I need to print the labels out. Usually, CIVAS is for antibiotics except Augmentin, ampicillin, cloxacillin and c-pen. These 4 antibiotics are not stable to make CIVAS.

6. At 12pm and 4 pm, I will call satellite to ask if the discharge medications are ready or not as I need to send those medications the the patients. 

7. Some patients need counselling on certain instruments or medications such as insulin pen (Novopen), MDI (metered dose inhalation), warfarin or others. The most common counselling in ward are insulin and MDI.

8. Clerk case is one of the requirement in clinical dept. For adult ward, 20 cases per week while for paeds 15 cases per week. For ICU, 15 cases per week. Thus I have to fill in the CP2 and CP1 form everyday to make sure I have met the requirement. I will usually stay back after work to do the clerking until 7 or 8pm.

Liku-liku Melancong


Assalamualaikum wbt dan salam ceria semua..

Hari ni nak cerita pasal pengalaman pahit getir ketika melancong di luar negara. Maklumlah, walaupun tempat pelancongan tu maybe zero-percent crime rate, tapi kita tidak boleh pandang remeh tentang keselamatan ketika berjalan-jalan ye. Walau ke mana pun kita melancong, kena sentiasa berhati-hati dengan keadaan semasa serta dengan orang sekeliling. Kadang-kadang orang yang tak suspicious tu la yang akan merobek kocek anda.  



1. Rome, Italy

Italy memang terkenal dengan kadar penyeluk saku yang tinggi ye. Kena sentiasa berwaspada pada setiap masa. Ada beberapa kes, kawan-kawan kena seluk tanpa disedari. Walaupun beg sentiasa berzip, kadang tangan pencuri ni pantas. Zap zap zap terus hilang dompet dan segala. Peristiwa ini juga berlaku kat ayah aku semasa menaiki Metro (subway) di Rome. Tetiba dihimpit orang dan lepas tu terus sedar wallet dalam kocek sudah pun lesap. Maka habislah wang Euro & Pound Sterling, kad pengenalan, kad kredit dan sebagaikan. Memang terus gelabah, lepas tu semua orang takde mood nak jalan2 dah. I won't come here again. Menyampah... Habis sumber kewanganku tau, kena berjimat cermat je lepas tu.



2. Venice, Italy.

Wahhh, Italy lagi. Kau memang bad luck ke apa? Selain kejadian curi-mencuri, negara Europe juga terkenal dengan mogok aka strike. Peristiwa yang mungkin terjadi adalah seperti mogok kenderaan awam mahupun mogok lapangan terbang. Kalau ini terjadi, sistem kenderaan akan lumpuh sebahagian besarnya. Semasa di Venice, during our last day of trip in Italy, perkerja2 Batobas (bot) bercadang untuk mogok selama satu hari. Maka kami terpaksa berjalan kaki dari hotel ke stesen kereta api yang jaraknya mungkin 3 km dengan mengusung beg, jalan celah2 bangunan dan juga turun naik tangga. Oh, I hate it.. Sampai kat stesen je terus beli air..



3. Brussels, Belgium

Ini pun berkaitan dengan mogok kenderaan awam. Metro memang lumpuh sepenuhnya manakala bas sedikit sahaja yang beroperasi. Maka berjalan la 2km dengan beg ke hotel dari stesen bas utama. 40 minit jugak la. Tak ke membazir masa tu? Lepas tu nak pergi city balik kena la jalan 2 km lagi. Ahhhh, benci.. Pembaziran di situ.



4. Paris, France

Ok, ini bukan la liku-liku sangat. Tapi nak cerita jugak. Ni pun berkaitan dengan kenderaan awam iaitu bot tambang. Ni memang salah sendiri, di mana kau kehilangan one-day tiket. Ish3, dalam 9 Euro camtu la tiketnya. Masa tu tengah busy dok shopping souvenir, mungkin tiket tu terkeluar dari wallet. Natijahnya, aku sorang naik metro ke Louvre manakala kawan aku lagi 3 orang pergi naik bot tu, juga untuk ke Louvre. The bright side is, aku sampai lagi awal daripada diorang. So, aku sempat jalan2 kat garden depan Louvre tu. Maunye 1 jam aku tunggu kot diorg tak sampai. Time aku tengah lepak2 makan roti, tetiba banyak burung merpati datang nak mintak roti. Kacau betul la.. =.=



5. Nevis Range, Scotland

Ski trip tak menjadi. Dah semangat tau pergi ke Fort William sebab nak main ski kat Nevis Range. Masa naik bas 4 jam dari Glasgow ke Fort William tu pun dah cukup mencabar. Met Office dah keluarkan warning angin kencang ribut taufan. Ok, itu terlebih sudah. Takde taufan pun, cuma angin yang sangat kuat. Banyak kali juga la dengar dentam dentum bas tu kena pukul angin. Memang sangat kuat. Alhamdulillah sampai juga ke destinasi. Dekat atas gunung tu pulak, kitorang plan nak main sledge dulu lepas tu baru ski. Masa tengah sledge tu memang ribut dan melibas2 muka aku. Dengan hujan batunya lagi, pedih gila muka. Maka rehat la kejap kat kafe dia sambil bantai hot chocolate. Lepas tu konon nak sewa peralatan ski kat bawah. Pastu orang tu cakap tak boleh sebab tempat ni dah nak tutup due to bad weather. Cis cis cis...




Ok, itu sahaja yang teringat buat masa ni. Kalau ada tambahan nanti, aku update la yeee...

PRP Week 17 - Outpatient (OPD)


Yuhuuuuu, well last week was quite hectic in OPD. This was due to the celebration Chinese New Year a week after that thus a lot of patients came to hospital to take their daily medications. However, I wasn't really affected by that mainly because I didn't really work in OPD last week. I was assigned to work at Oncolocy Pharmacy and Emergency Department for the whole week.

Working in Onco Pharmacy wasn't very busy, at least not as busy as Farmasi Klinik Pakar. In morning, I helped Kak Syahida doing the filling and sometimes with the dispensing. This pharmacy focussed more on Oncology patient, thus there are many cytotoxic drugs here. Some common drugs use in treating cancer patient include mercaptopurine, imatinib, capecitabine and many others. Some hormonal drugs include tamoxifen, letrozole and anastrozole. Everyday, I will prepare the discharge medication for Day Care, which later the CDR pharmacist will collect them and give to the patient. Besides that, I also did some repacking. Packing metoclorpramide to 15's or 10's and dexamethasone to 3's, 6's or 12's. The Onco pharmacist only opens from 8am - 1pm. Thus, after that, patients will go to Farmasi Klinik Pakar to collect their medications.

After lunch break, I will open the Emergency Dept Pharmacy at 2 pm. I worked alone here as usually there are not so many patient here. The medications are usually for the common sickness such as fever, cough, flu, asthma, accident and food poisoning. It's quite leisure working here, but it's bored because didn't have friends to talk to. So cannot gossip2 here and there la. I worked until 6 pm and after that, other pharmacists will replace me from 6 to 10pm (evening shift) and also from 10pm to 8am (night shift). So, ED pharmacy will close at 8am.

Legoland Malaysia Part 2: All the Fun


My second visit to Legoland was with my family, aunt, uncle, cousin and grandma. Nenek ikut skali kau.. Ada nenek kau mampu? Hahaha... This time, our main goal was to play all of the rides. We arrived at the entrance a few minutes before 10am. There was already a tonne of people, eager to get into the park. The weather was okay, but we brought umbrellas along just in case it rains.

Sediakan payung sebelum hujan gitu. It was raining few times during our stay inside the park. Sometimes it hot and bright, and suddenly the weather changed drastically.

Lego Technic

Our first ride was Project X which is a 4-seated roller coaster. It's doesn't look very thrilling but once up they. I felt like I was nearly threw out from the coaster. It was quite high speed and the inertia was high as well. Memang rasa macam nak tercampak keluar ke kiri ke kanan. Hehehe.. Don't underestimate this ride ok.. Tapi masa ni beratur agak lama la.. dalam 45 minit gitu.. Pity my parents and my grandma because they had to wait outside for so long.





We played another ride here which was Aquazone Wave Racers. This ride is basically a spinning boat-like structures where we spun a few rounds embracing the water splash and water bomb. It was quite fun although this was meant for children. Hahaha..


Lego Kingdom

Here lies a couple more of roller coaster. My brothers and I rode on The Dragon roller coaster once and The Dragon's Apprentice twice. The later is for children actually although both of them are dragon-themed. Since we rode this quite late at the evening after the rain stopped, thus we queued only for a few minutes due to many people had went back.



There's a souvenir shop here as well inside the castle-like building. Our cousin, Siti played The Royal Joust ride which is a ride on a Lego horse.


Imagination

We watched a 4D show here. A story of Clutch Power who is one of the Lego legends. We queued for about 30 minutes before entering the hall. The show was nice. We got sprayed with water several times although the seats were static. I was hoping for the seats to move actually. Other effects includes strong winds and foams.

We also took the chance to board the Observation Tower. The tower spun slowly above the air and we could see the whole Legoland. Legoland isn't too big after all. We could observed several constructions were still going on.



Pizza Mania

We had our lunch here. Although the food are kinda pricey here, we still had to eat here because our stomachs had been growling so much. Hehehe.. We bought several sets of pizzas and pastas. Pasta sets cost around RM13 which was quite affordable. It includes pasta, soup and a drink.



Land of Adventure

Here got one ride that all of us eager to try, Dino Island! It's wet ride, so be prepared with rain coats or whatsoever. We didn't put on any, thus the result was severe. My brother suffered from the wetness of the water as he sat at the back. Lucky me because I sat in the middle. The slides was steep and the splash was thrilling.

In Lost Kingdom, we sat on a moving car and then shoot the red marks with our laser guns. At the end of the game, the total score was shown and we could compare the scores with each others.

Lego City

We spent about half an hour watching a live performance. It was very exciting at the beginning but later became quite lame. It's about how to be safe inside your own house.



Mak Induk and Siti played at the Lego City Airport which is a spinning planes.


Miniland

Our final stop was the Miniland where a lot of historical monuments from Asia are displayed. They are made from Lego bricks.




The Big Shop

As we were waiting for the rain to stop, we had a shelter outside The Big Shop. I, in the other hand roamed inside the shop thinking what to buy as souvenirs. I bought a few more random Lego Minifigures and a few magnets. They are generally very expensive. So I had to close my eyes when purchasing them.

Legoland Malaysia Part 1 : Miniland


Yuhuuuuu, I'm so happy finally I got the chance to step my feet into the brick-world, Legoland! Legoland Malaysia (LLM) is located in Nusajaya, Johor. It took approximately 30 minutes to reach there, by driving from Johor Bahru. It's gonna be exciting. 

Oh yeah, I'm gonna blog it in two entries as I went there twice (three times actually including the preview with my best friend, Lukman). Hehehe.. The daily pass cost RM140 (walk in) and RM110 (bought online 7 days prior to admission). However, I bought the annual pass which was at that time only cost RM275, now the price jump to RM325. Pengsan kau.. Mahal gilaaaa... Alhamdulillah dah beli awal2.. :)

So during my first official visit with my family, we didn't really play the rides, but just to wander around the area surveying and admiring the structures. So we spent most of our times in Mini-Land where the miniatures of Asian countries' landmarks were made from Lego bricks. It's quite stunning actually. Very nice indeed.

Legoland is divided into 7 areas:

3. Lego Kingdom
4. Imagination
5. Land of Adventure
6. Miniland
7. Lego City





For this visit, we only focussed on Miniland.and also boarding the Legoland Express train. Enjoy the pics! :)

6 Corner Senibong Seafood


Senibong is located aprroximately 10km from Johor Bahru city centre. It is a place where a lot of seafood restaurants are located. Our choice this time was 6 Corner Senibong Seafood. This one is quite famous because it was featured in a Malaysia TV program, Jalan-jalan Cari Makan (JJCM). This restaurant is located in the middle of the street.

The view was great from here, we could see the glittering Singapore city at night, across the Selat Tebrau. Ok, forget Singapore and let's focus on the food menu. I was here with my aunt, uncle, grandma and cousin. They belanja of course. :)

So we ordered butter prawn, battered cuttlefish, ikan tiga rasa, ikan steam and kailan ikan masin. While waiting for the food to arrive, there were people from small stalls sending kacang rebus and otak-otak to our table. The otak-otak is very nice although I found it quite pricey. I think everything here are quite expensive. 

The food was nice and delicious. I like the crunchiness of the butter prawns. I am myself is a seafood lover. Just throw me anything, I will eat them happily. Hahaha.. The fried cuttlefish was good too but tough a bit. The fishes were great too. I love collecting the pineapples in the sauce and finishing them all. :)

PRP Week 14,15 & 16 - Outpatient (OPD)

Yuhhuuuuu, I'm back.. It had been a while since I last blogged about my PRP life. 

So, for the last 3 weeks, I was still attached to OPD, doing filling and dispensing like usual, plus with another responsibilities which I was assigned to take care of Dangerous Drugs (DD) and also Sistem Pendispensan Ubat Bersepadu (SPUB) for two weeks.

DD is collectively a term used for dangerous and psychotropic substances such as diazepam, morphine, phenobarbitone, clonazepam and others. This type of medications need to be stored in a double-locked area; one is the room door and another one is the metal cabinet. This cabinet need to be locked 24/7 unless I need to take out some for the patients. First thing to do when getting a DD prescription is to print out the presciption or make a copy of it, make 2 labels, record in DDA book and finally take it out from the metal cabinet. It's a simple step, but quite tedious especially when you got a few DD prescription at a same time.

SPUB is a term used when patients from other hospital are referred to our hospital and this is called SPUB masuk. For our patients which are referred to other hospital, it is called SPUB keluar. Some medications which are not available in our hospital sometimes need to be indented from the respective hospitals.

So, when there is a SPUB keluar, pharmacists who did the dispensing will shout SPUB! SPUB! Then, they will hand me the patient's card where the name of hospital/klinik kesihatan was writter. I will print 2 copies of the prescription and then fill in the SPUB forms. One copy will be given to the patient and one will be kept.

Ahhh, finally I have finished my 2-weeks role taking care of DD and SPUB. Next week, I'm gonna work in Onco Clinic and Emergency Department for a week. So my working hour will be from 9am to 6pm. 

I attended 5S meeting last week. It's about a program which was created to improve out working environment. 5S stands for Sisih, Susun, Sapu, Seragam and Sentiasa Amal.
